How to print Medical Release Forms

1. Login to your account
2. The screen you will see is, MEMBERS HOME 
3. Click on PAST REGISTRATIONS
4. Your child's name pops up
5. Next to child's name there is a PRINT FORM box
6. Click there and the medical release form prints
